By completing the form below, I understand that Healing Presence is a simple, gentle, energy technique that is used for deep relaxation and helping the client’s whole system (mind, body and spirit) to align to it’s own wisdom and self healing abilities.

I understand that Maddie Mayfield does not diagnose conditions nor prescribe or perform medical treatment, prescribe substances, nor interfere with the treatment of a licensed medical professional. It is recommended that I see a licensed physician or licensed health care professional for any physical or psychological ailment I may have.

I understand that Healing Presence can complement any medical or psychological care I may be receiving. I acknowledge that long term imbalances in the body sometimes require multiple sessions in order to facilitate the level of relaxation needed by the body to heal itself

I understand that Healing Presence is an energy healing methodology that can include if requested, the laying on of hands to a person who remains fully clothed. There will be absolutely no touching of breasts, genitals or buttocks

I understand that Healing Presence is being provided by Maddie Mayfield at my request. I agree to hold Maddie Mayfield harmless and understand that she is not responsible for the outcome of the session.

I understand that Maddie Mayfield will keep any details about me and the sessions in total confidentiality, unless she feels that I present a danger to myself or to others.
Cancellation Policy:
Please note that once you have booked an appointment, it means that Maddie has reserved time in her schedule exclusively for you.
Therefore please do not cancel your appointment less than 24 hours before it is scheduled to take place.
If you have previously agreed and given a donation to cover scheduled appointments that you cancel less than 24 hours in advance, then this donation might not be possible to refund. You can provide cancellation notice for your appointment via email at maddie@heartspring.co.uk or via phone or message at 0777 2000 876
I hereby certify that I have read this document and I understand its content. I am aware that this is a release of liability as well as a contract and I sign it of my own free will.
By signing this I give myself permission to advocate for myself to feel safe and comfortable while working with Maddie Mayfield.
